Have you ever been falsely accused of child-neglect and/or reported to the Children's Aid...?

Getting reported and being investigated with actions taken, are 2 separate things. Reports don't go on your permanent personal record. Investigation's and actions taken do. If you're innocent, don't worry about it. If you have done the things you have been reported for, you should be very concerned. (Hopefully you're not, for the kids sake)
